Torquay & District Medical Society meeting

If you have any special dietary requirements for your meal at our TDMS meeting, please use this form to notify the Secretary.
Vegetarian and Vegan options are available for the meal but must be pre-booked. Please use this form to request your alternative meal.
Other requests may be possible but cannot be guaranteed.
Please complete all the contact fields in case we need to discuss your meal options.
If your place was booked as part of a group, please tell us the name of the person who booked so we can locate you on the night.
All dietary requirements must be notified in advance. The catering team is not able to accommodate requests made on the night.
